The following is a brief summary of the project in which you are asked to participate. Please read this information before signing the statement below.

TITLE OF PROJECT: Self-Compassion, Perceived Social Support, & Attitudes toward Addiction

PURPOSE OF STUDY/PROJECT: The purpose of this study is to better understand self-compassion, perceived social support, and attitudes toward addiction.

PROCEDURE: Participation in this study is voluntary and you must be 18 years of age or older to participate in the study. You can quit participating at any time without penalty. This survey should take approximately 20 minutes to complete.

INSTRUMENTS: In the survey you will be asked to provide some basic demographic information. Additionally, the survey includes a collection of items that will assess qualities such as self-compassion, perceived social support, and attitudes to addiction.

RISKS/ALTERNATIVE TREATMENTS: The participant understands that Louisiana Tech is not able to offer financial compensation nor to absorb the costs of medical treatment should you be injured as a result of participating in this research. Although we do not expect completing this survey to be highly distressing, you are encouraged to contact a local mental health professional (e.g., psychologist, psychiatrist) if you experience significant distress while completing this survey. The following disclosure applies to all participants using online survey tools: This server may collect information and your IP address indirectly and automatically via “cookies”.

EXTRA CREDIT: If extra credit is offered to students participating in research, an alternative extra credit that requires a similar investment of time and energy will also be offered to those students who do not choose to volunteer as research subjects.

BENEFITS/COMPENSATION: No compensation will be offered to participants by the researchers. However, participants who complete the survey may find the experience enjoyable as positive personal characteristics and experiences will be assessed.
