Screening Questionnaire

Thank you for your interest in sharing your experiences with how you learned caregiving tasks required to take your infant home from the hospital. This study aims to understand the process that caregivers of infants with congenital heart disease, who needed surgery after birth, go through to become experts of the infant’s care in the first year after your child’s initial hospitalization. Please answer the following screening questions to make sure you meet the qualifications for the study:

How did you hear about this research study?

Are you a primary caregiver for your infant?
A primary caregiver is a parent or significant relation who takes care of the infant, learned the care tasks necessary to take the infant home from the hospital, and is responsible for the ongoing child-rearing tasks.
