Autism Spectrum Disorder Diagnosis Age in Relation to Race/Ethnicity and Gender and Its Impact on Mental Health

INFORMED CONSENT

Rider University is committed to protecting the rights of human subjects who wish to participate in research. The following information is provided for you to decide whether you wish to take part in this study. 

  1. Purpose of the Study
    This project includes a questionnaire assessing how the relationship between the ages of individuals with Autism Spectrum Disorder were diagnosed, their gender, race and ethnicity, and how these factors may have impacted their mental health. 
  2. Procedures and Time Commitment
    The study should take approximately 15 minutes to complete and you are required to read and sign this form, should you agree to participate in this study. 
  3. Risks and Benefits
    There are no experimental manipulations nor deception being used in this study. There are also no foreseeable immediate or longer-range risks expected from participation. The anticipated benefits are to look into how early support systems may be helpful to those with ASD and bring awareness to the disparities in diagnoses across different demographic groups.
  4. Confidentiality
    All responses will remain anonymous and no identifying information will be collected.
  5. Voluntary Participation
    You are not required to participate in this study and for any reason you can opt out at any time. Withdrawal will not lead to any penalty or loss of benefits to which you are otherwise  entitled.
  6. Approval
    This study has been approved by Rider University’s Institutional Review Board(IRB), the committee that oversees research studies where human participants are involved.  

I have read this Consent and Authorization form. I have had the opportunity to ask, and I have received answers  to, any questions I had regarding the study. I understand that if I have any additional questions about my rights  as a research participant, I may email the faculty sponsor of this project, Dr. Costello, at [email protected], or email Dr. Brosvic, a representative on the IRB, at [email protected]. I am aware that if I am struggling with any mental health discomforts brought about due to the questions in the survey, I am able to reach out to the National Alliance on Mental Illness HelpLine at 1-800-950-NAMI(6264), text NAMI to 62640, or email them at [email protected] for one-on-one emotional support.

I agree to take part in this study as a research participant. By my signature I affirm that I am at least 18 years  old and that I have read and had the oppurtunity to download a copy of this Consent and Authorization form.
